Investigation Summary
At approximately 5:15 p.m. on September 2, 2008, Employee #1 and Coworker #1 were cleaning up the fabrication shop. After they had picked up all the stone pieces the coworker helped load a wooden crate onto the forklift that the employee was driving. The employee drove out of the shop, down the ramp and around a corner very fast and the forks were really high. Coworker #1 watched as Employee #1 turned the forklift really hard and it fell over. The coworker ran to the employee and saw that his head was underneath the top of the forklift. The coworker lifted the forklift as high as he could and the employee was struggling to get up. Blood was coming from the employee's ear and mouth and he was groaning. The coworker ran to get help and emergency medical services were called. The coworker ran back out to the employee and put paper towels on his head wounds. The employee was gurgling, but never spoke. The ambulance arrived at the scene and Employee #1 was pronounced death at 5:39 pm.
