Employee Falls from Ladder, Suffers Concussion, Later Dies

At approximately 2:30 p.m. on May 6, 2009, Employee #1 was standing on the fourth rung of an 8-foot ladder applying plastic "wrap around" labels to HVAC piping. Other workers heard a noise and found Employee #1 lying on the floor. Employee #1 was transported to a local hospital which was just across the street from the site. Employee #1 was placed on life support but was taken off life support at approximately 10:00 a.m. on May 5, 2009, and died shortly thereafter.
