Investigation Summary
On May 6, 2009, Employee #1 was inspecting a 12Kv linebay. The employee opened the door to cabinet and placed his hands inside of the cabinet. The employee sustained an electric shock. The employee had his left index finger and knuckle amputated. He also sustained burns to his right index finger and right bicep. Employee #1 was hospitalized. The employee was wearing a flame retardant shirt, cotton shirt, leather palm gloves, hard hat and safety glasses at the time of the incident. Prior to beginning work on the equipment, the employee did not test to verify that it was deenergized.
