Employee Injurs Finger in Conveyor

On August 4, 2010 Employee #1 was working on a bagger machine and had finished changing the conveyor belts, removed their lockout tagout locks, and turned the machine back on. Employee #1 was putting away the tools, and had bent over to pick up the grease gun, when they noticed the one conveyor belt looked a little misaligned. Employee #1 reactively reached down with his left hand and pushed on the conveyor belt to see if it had in fact slid over. As Employee #1 did this, the conveyor belt's metal webbing, caught his hand and his left ring finger went in between the two conveyor belts causing a finger tip amputation.
