Investigation Summary
On August 9, 2010, Employee #1 was wiping the photo eye of a conveyor that moves cases of product. Apparently, Employee #1 had reached through the safety door in order to do this and had not locked out the machine. The machine was down at the time that Employee #1 was wiping the photo eye, but then turned back on. As the machine turned back on, something caught Employee #1's thumb. Employee #1 did not know what caught his thumb. Employee #1's left thumb was smashed, and part of the flesh was removed. Employee #1 was treated and released from the hospital.
