Employee Fall from Ladder and Fractures Hip

On June 25, 2010, Employee #1 was cleaning the top and lid of 5000-gallon tanks, which contained olives. He was not directed to enter the tanks, just the top and lid. The tanks are a non-permit-required confined space hazard. Employee #1 entered a tank to possible unplug the drain in the tank. He properly descended down into the tank with a harness and lanyard. As per the policy, Employee #1 disconnected the lanyard at four rungs from the bottom of the tank, when he fell and fractured his hip. The fire department was summoned and he was removed from the tank using a high angle pick procedure with a stokes basket. Employee #1 was taken to Tracy Memorial Hospital, where he received medical attention for a fractured pelvis and was hospitalized.
