Employee Catches Foot, Requires Amputations

On April 18, 2009, Employee #1 was operating a standing forklift. He stepped off the vehicle, while it was still moving. His foot became caught between the forklift and a wall, crushing his toes. He was transported to a medical center, where he underwent treatment, which required amputating the injured toes and was hospitalized.
