Employee Slips and Falls When Stepping on Pile of Wood

On October 23, 2008, Employee #1 was descending a ladder and stepped onto a stack of wood that was 4 in by 8 in. The wood shifted, and Employee #1 fell approximately 30 in. Employee was taken to the hospital, where he was treated for his injuries.
