Investigation Summary
On September 17, 2008, Employee #1 was cutting up a truck crane for scrap metal. He was lying under the rear of the truck cutting the downrigger behind the rear wheels. When the cut was completed, the counterweight on the rear of the truck dropped on his legs and torso, killing him.
