Investigation Summary
On September 19, 2008, Employee #1 was working on the flat roof of a two-story storage warehouse. The employee was assisting coworkers with waterproofing the skylights by gathering tools and equipment for them. As he was cleaning up the area of the roof, he stepped through the skylight opening and fell 23 ft to the concrete floor of the storage warehouse. Employee #1 was killed.
