Investigation Summary
At approximately 11:45 a.m. on July 28, 2008, Employee #1 and #2, electrician's helpers, were installing conduit and electrical outlet boxes in the masonry wall of a concrete stairwell for the parking deck that was under construction. Employee #1 and #2 were working from the cantilevered platform attached at the second level of a three buck high tubular welded frame scaffold. Unknown to the two employees, the left side support bracket was bent and gave way, causing them to fall 20 feet. Employee #1 incurred a severe sprain to his left ankle, and Employee #2 sustained a fractured pelvis, compressed disc in the lower back, and a broken left leg. They were both hospitalized for treatment.
