Investigation Summary
On July 16, 2008 Employee #1 drove to the construction site in a flat bed truck with a load of brick pavers and began to unload the brick pavers from the truck using a fork lift truck. According to the witness report, the noise of someone yelling attracted his attention and he ran toward the yelling. Employee #2, was found lying on his side with the cage of the fork lift pinning him underneath his chin in the neck area.
