Event: 10/17/2008
Employee Is Killed in Fall from Man Basket

At approximately 4:26 p.m. on October 17, 2008, Employee #1 and two coworkers were installing an auger in a grain warehouse. Employee #1 was working in an elevated man basket, approximately 17 ft high. The basket was sitting on the forks of a forklift but was not chained, bolted, pinned or otherwise attached to the forklift. One of the cables used to lift the auger failed, and the forklift operator backed away from the auger and attempted to lower the basket to the ground. The forklift operator engaged the wrong lever and tilted the mast forward, which caused the basket and Employee #1 to fall approximately 17 ft. Employee #1 was killed.
