Investigation Summary
On August 11, 2008, Employee #1 and a coworker were working on a carpenters bracket scaffold. The scaffold had no fall protection system and was not fully planked. The platform did not extend over the supports at least 6-in. Employee #1 and the coworker both fell from the scaffold. The coworker received superficial injuries and continued working. The coworker's injuries were not recorded. Employee #1 fractured both wrists.
