Employee Is Struck by Falling Plywood, Injures Leg

At approximately 2:00 p.m. on April 30, 2010, Employee #1 was helping coworkers to move a metal scaffold. During the moving, a sheet of plywood fell off the scaffolding and hit Employee #1 in the leg. The Division was notified of the accident at 2:03 p.m. on April 30, 2010 by the employer and initiated an inspection on May 4, 2010. The facility involved in the accident was a concrete contractor, making forms, pours, places, finishes and installs specified mass, pavement, flat and other concrete work. Interviews were conducted of employee's supervisor, injured and other employees. The investigation concluded that no violation of any Title 8 standard in connection with this accident as a result, the Division did not issue serious accident-related citations. Only no accident related citations issued.
