Employee's Fingers Are Crushed by Block

On February 2, 2010, Employee #1 was working in the CNC machining area. The CNC machine (Shoda Iron Works Co., Model Number NCS16U-1634, Machine Number YO2A71001) shapes Ren Block modeling board so it can be used to make a mold. Employee #1 placed a small support block on the CNC table and a coworker, a forklift driver (Misubishi 35-2; Model Number FG18N, Serial Number AF3400422), placed the Ren Block on the table on top of the support block. The coworker then pulled back the forks so 12 inches of forks supported the Ren block. The coworker then tilted the forks slightly so Employee #1 could pull out the support block. The Ren Block slid off the forks of the forklift and crushed the fingers of Employee #1. Injuries of Employee #1 included amputation of the first joint of the third digit of the right hand and tissue damage of the forth digit of the same hand. Based on the dimensions of the Ren Block, a 450 Block, by CMT Materials, Inc., it weighed approximately 460 pounds. The employer, an aerospace manufacturer of insulation products reported the incident on February 3.
