Investigation Summary
On April 4, 2009, Employee #1 Employee #1 was instructed to assist in cleaning an automobile while it was still inside the tunnel of the car wash on a conveyor system. The conveyor was not "on" at the time of the incident. As Employee #1 was cleaning the rims of the automobile with a rag, the conveyor was switched on by an unknown person, causing the vehicle to move forward. As a result, Employee #1's right hand was cut by the rim of the vehicle, causing him to sustain an amputation of the right ring finger tip. Employee #1 was hospitalized.
