Investigation Summary
At approximately 10:45 p.m. on April 21, 2009, Employee #1, a heavy equipment operator, worked for a company which provides concrete cutting, breaking, excavation and highway grinding services. Part of his job including greasing head bearings, cleaning spray bars, and checking and inspecting the belt's tension. He was driving a PC-6000 EC highway grinding machine Number 197, Diamond Products, in the attempt to grind a highway. He tried to check the tension on a drive belt. He disengaged the main friction clutch belt drive without turning the engine off. He reached around, behind the belt guard and grabbed the belt below the entrance side of the drive pulley. The free floating grinding head began to rotate and pulled his hand into the pulley. He suffered a crushing injury to his right hand and an amputation of his right finger. Employee #1 was hospitalized.
