Employee's Legs Are Fractured Under Falling Forklift Load

On December 11, 2008, Employee #1, a maintenance mechanic, was working at a large municipal wastewater treatment center. Three identical variable frequency drive motor control panels were being loaded onto the bed of a flatbed carrier. The panels measure approximately 7.5 ft by 5 ft by 22-in. A large lift was utilized to transport the panels to a carrier. The first panel was successfully transported on the forks of a Clarklift 12,000 pound capacity forklift, Serial Number CY120-45-750-866. The forks were 6 ft long. While loading the second panel on to the forks of the forklift, the panel struck the ends of the fork. The panel slid along the forks with the bottom of the panel sliding away from the lift and towards Employee #1. The underside of the panel struck Employee #1 and both of his legs were pinned under the panel. Employee #1 was hospitalized with two fractured legs.
