Investigation Summary
At approximately 1:00 p.m. on November 11, 2008, Employee #1, a maintenance worker, was standing on a 5 ft A-frame ladder to reach the fascia of a second-floor balcony. He was using a screw driver to pry the fascia free. While he was removing the fascia, he lost his balance and fell 3 ft to the ground, fracturing one of his hips. Employee #1 was taken to Kaiser Permanente Hospital in Woodland Hills, California, where he was hospitalized and treated for his injury.
