Investigation Summary
On November 05, 2008, Employee #1 was seriously injured as a result of a fall. On the day of the accident, Employee #1 was cleaning a layer of dust on some lights using an 8 foot ladder and a vacuum cleaner. Employee #1 lost his balance when he pull on the vacuum hose and fell 6 feet hitting his head on a concrete floor.
