Investigation Summary
At approximately 9:00 p.m. on October 28, 2008, Employee #1 was working at a roofing supply manufacturer as a Catcher Operator (person ensuring roofing bundles are processed through the conveyor system). At the time of the accident, Employee #1 was performing a regularly assigned task of observing roofing bundles pass through conveyor system without delay, clearing potential jam of bundles and stacking them. Prior to the incident, Employee #1 observed several bundles accumulate on the Hi-Lug conveyor system, which runs perpendicular to the timing conveyor. There are two timing conveyors parallel to each other. Employee #1 was standing on the west side of timing conveyor Number 2 and Number 4. The bundles jammed on the Hi-Lug conveyor. Employee #1 powered the Hi-Lug conveyor off but did not power off the timing conveyors closest to him. Employee #1 reached over to grab the bundles, and his shirt became caught in a small opening. Employee #1's right triceps muscle was pulled in, resulting in friction burn to the area. Employee #1 suffered burns to his skin due to having his upper arm caught in small opening of machine. Employee #1 was hospitalized.
