Investigation Summary
On May 16, 2008, Employee #1 was a woodworker at Custom Design Woodworking. He was using a table saw to dado small pieces of wood. He cut a dado approximately 0.25-in. from the edge of one piece. He needed to dado another piece. Instead of adjusting the guide plat to the other side, he placed the piece of wood ahead of the saw and moved the wood backward. He was injured when the wood kicked back and his left hand went into the saw blade. Employee #1 was hospitalized at West Hills Medical Center for surgery and treatment of the partial amputation of his left thumb, fractures of his thumb and left ring finger, and tendon damage.
