Investigation Summary
On March 5th, 2011 Employee #1, a construction laborer, of Jacobsen Construction Inc., was assigned to remove and sweep the grout fallen to the pavement and to clean the building. Employee #1 was working alone, so when Employee #2 came to pick him up at the end of the shift, Employee #1 was found face down on the floor near the southwest corner of the building. Employee #2 could not arouse him so he called Employee #3, a superintendent, who also called 911. The EMTs checked for vitals and found him nonresponsive. Employee #1 was taken to Dixie Regional Hospital where he was officially pronounced dead. He was taken by the Medical Examiner to determine the cause of death. The Medical Examiner observed that Employee #1 died from coronary artery atherosclerosis.
