Investigation Summary
At approximately 1:20 p.m. on January 7, 2009, Employee #1, a general laborer, was working at a residential construction site. After returning from his lunch break, Employee #1 mounted a site-built scaffold that had no guardrail to continue his assigned duties. As he was working, Employee #1 fell 13 feet to the ground, striking his head on a landscaping rock. Emergency services were contacted and Employee #1 was taken by helicopter to a nearby hospital, where he died.
