Investigation Summary
On October 10, 2008, an employee was adding chemicals to a vessel, paddle blender, to be blended. The paddle blender had a horizontal auger that mixed the chemicals in the 48-in. by 113-in. vessel. The employee fell in, slipped or lost his balance and fell through the 24-in. by 24-in. opening. A coworker heard a scream and ran to turn the power off on the paddle blender. The employee had been caught in the horizontal auger of the paddle blender. The employee was killed.
