Employee Is Caught in Bucket Elevator Belt, Is Killed

Employee #1, a maintenance man, was repairing a bucket conveyor that had malfunctioned. The work was to the point that the machine was placed in the jog mod. Employee #1 subsequently went inside the elevator housing. For some reason the elevator started moving with Employee #1 still working on it. Employee #1 was caught between the bucket elevator belt and the main drive pulley and was crushed to death.
