Investigation Summary
Four employees were stacking trusses against some poles as they were being built. The stack was tied back to the poles to hold them upright. As another truss was built, it was carried out and added to the stack. The two employees at the center would hold it up against the stack, while the other two employees at the ends would untie the rope from the stack and tie-in the truss that had been brought out. At the time of the accident, there were 10 trusses stacked and tied to the poles. The employees had brought out another truss and had laid it up against the stack. Again, the two employees at the center were holding up the truss against the stack, the two employees at the ends untied the rope on the stack to tie in the new truss. At that time for some reason the stack became unstable and started falling towards the employees. The two employees at the ends were able to get out of the way. Employee #1, one of the employees that had been holding up the truss, was struck on the head by a gable as the stack fell, and the entire stack then fell on him. The other employee had the gables fall on either side of him and was not struck by the stack. Employee #1 was killed.
