Employee Dies after Being Struck by Furniture Box

On January 19, 2010, Employee #1 was unloading a trailer. An elevated box containing a bookcase tipped forward and struck Employee #1 on the head. He was treated and released from the hospital on three occasions following the injury. He died on February 2, 2010.
