Employee Amputates Fingers in Press Slide

An employee was having problems with a die-cast press. The slide was filling up with metal, preventing the slide from returning to its original position. A second employee assisted the first employee in attempting to fix the problem. The second employee wiped the cylinder with the fingers of his right hand, and the first employee pushed the core button, which activated the slide. The slide caught the second employee's fingers. The injured employee suffered amputations of the tip of his index finger and the end of his middle finger to the first knuckle.
