Emplolyee's Ankle Is Fractured by Forklift Counterweight

On August 26, 2010, Employee #1 was replacing a muffler on a forklift #1. He inserted the forks of forklift #2 into the counterweight of forklift #1. He lifted the counterweight up and backed up approximately 3 ft from forklift # 1 without securing the counterweight to the carriage of the forklift # 2. He then descended from forklift # 2 and as he was walking by, the counterweight fell off from the forks, spun and a section struck his right ankle. His right ankle was fractured. Employee #1 was hospitalized for several days.
