Employee Falls from Ladder, Sustains Concussion

At approximately 1:00 p.m. on July 29, 2009, Employee #1 was standing on the third step from the top of a six-foot, A-frame, portable ladder. The ladder was positioned inside a refrigerated warehouse storage room. Employee #1 was installing a clear plastic debris curtain inside the room. The debris curtain was being installed on the inside of the entrance opening to the refrigerated room. The height of the entrance was found to be 10 ft from the concrete floor. The ladder slipped out from under Employee #1 while he was standing on the ladder. Employee #1 struck the concrete floor with his head. No bruising or contusions were observed. However, Employee #1 did sustain a concussion. According to witnesses, Employee #1 did not lose consciousness. Employee #1 initially declined medical treatment, however, a few minutes later he requested medical treatment. Employee #1 was hospitalized.
