Employee Falls from Same Grade Onto Machinery/equipment

At approximately 1100 a.m. on April 18, 2009, Employee #1, an apprentice operator, was swabbing the floor in a section when his pant leg cuff caught a valve handle. It caused him to stumble, striking a 30 lb air supply pipe next to him. Employee #1 sustained three broken ribs and was hospitalized for three days before being transferred to a nursing home for care.
