Investigation Summary
On October 7, 2008, an employee was standing inside the container while a coworker was operating a forklift. As the forklift entered a container, the operator saw the employee standing inside the container. The coworker stopped the forklift. However, the front end attachment of the forklift struck the employee's right foot, fracturing two toes. The employee was admitted in Highland Hospital for observation until October 10.
