Investigation Summary

At approximately 2:45 p.m. on October 8, 2008, an employee was standing on fold up chair while she was placing a plastic over a laundry bin. As she was descending, the chair started to fold up and she lost her balance and fell to floor. The supervisor ran to the employee's aid and called emergency medical services. The employee was transported to to St. Louise Hospital in Gilroy, CA, where she was treated for sprained back and wrist. She was not hospitalized.
