Employee Falls from Scissor Lift and Is Killed

On June 26, 2008, Employee #1 was operating a Skyjack R-3155 Scissors Lift (Serial Number 711406, Model Number 4632). He was maneuvering the lift, when he struck part of the racking system bowing the end rail, damaging one of the top rails on the lift. The employee was found on the ground, with serious head and body trauma from the fall. A telescoping top rail adjacent to the one that the employee had hit was separated and dangling when the employee was found. Employee #1 was killed.
