Investigation Summary
On May 6, 2008, Employee #1 was operating a Delta brand 10-inch tilting arbor table saw. He was cutting Formica brand laminate while manufacturing a counter top. There were no guards on the table saw. The employer stated he does not do any training on the saws. When he hires employees, he hires only qualified employees. Employee #1 did not appear to have adequate training. The thin Formica "jumped" and Employee #1's hand touched the blade, resulting in the amputation of the pinky, index finger, and the thumb of the left hand. The middle and ring fingers were severely injured, but were not amputated. It was not known at the time of this report if they will have to be removed later. Employee #1 was taken to Trinity Lutheran Hospital in Ft. Dodge Iowa, and later transferred to Mercy Hospital by ambulance in Des Moines, Iowa. Then Life Flight took him to the University of Iowa Hospital in Iowa City for treatment.
