Employee Falls from Elevation, Later Dies

On April 16, 2008, an employee fell into excavated area next to residential formwork striking his head. The fall was not witnessed. The employee could have been walking on the formwork and fell approximately 9-ft., or the employee could have been walking next the excavated footing area and fell approx 5-ft. The employee was transported to Mercy Hospital in Sioux City, IA, where he died three days later from brain trauma.
