Investigation Summary

On June 15, 2010 at 4:45 pm, an employee (EE1) of JBS USA LLC dba Swift and Co. sustained a serious injury. The accident was not reported to Cal/OSHA by the employer. The Cal/OSHA reporting requirements were not met. The accident was reported by the fire department. JBS USA LLC dba Swift and Co. is a beef and pork processing and packing facility. The facility is located at 13215 Cambridge St., Santa Fe Springs, CA 90670. EE1 was working as a meat band saw operator at the facility. EE1 started his shift at 6:00 am and was trimming meat by hand until 1:00 pm. At 1:00 pm he took a break. At 1:10 pm EE1 started working on the AEW band saw (Machine: 400M, Identification number: 1, Serial number: LHS342097) to cut pork butts into chops. He was working alone. At 4:45 pm, he was cutting a pork butt. As the piece becomes smaller, the employee is required to use a push bar to keep his hands away from the point of operation. EE1 was positioning the smaller piece with his hand instead of with the push bar. While doing this his left hand moved across the blade. He sustained an amputation of the first and middle fingers of his left hand. EE1 stated that he should not have used his hand to position the meat. EE1 was not following procedures when cutting meat which caused the accident.
