Investigation Summary
At approximately 2:00 p.m. on December 30, 2009, Employee #1 was working for A-1 Best Food, Inc., a manufacturer of wholesale bread products. He was working at the bread bagging machine when one of the plastic bags became entangled in one of the chain and sprocket drives. As the employee reached for the bag to free it, his right hand was pulled into the in-running nip point of the chain and sprocket drive. He suffered a severe laceration to his right thumb and middle finger that required hospitalization. Further investigation revealed that the employer did not have a written hazardous energy control procedure; the employer failed to guard an exposed chain and sprocket drive; and the employee was not trained on the proper lock out and block out procedures.
