Worker's Hand Is Lacerated by Catching in Bagging Machine

At approximately 2:00 p.m. on December 30, 2009, Employee #1, a worker at A-1 Best Food Inc. in Montebello, CA, was working at a bread bagging machine when one of the plastic bags became entangled in one of the chain and sprocket drives. He reached for the bag to free it, and his right hand was pulled into the in-running point of the chain and sprocket drive. The employee suffered severe lacerations to his right thumb and middle finger and less severe lacerations to the other fingers. The lacerations of the thumb and middle finger required hospitalization for surgery. The employer did not have written Hazardous Energy Control Procedures for employees to follow. The employer failed to guard an exposed chain and sprocket drive system. The employee was not trained on the proper lock out/block out procedures and had not hit the stop button next to him before reaching into a running chain and sprocket drive before clearing a jam.
