Employee Is Killed Run over by Industrial Truck

At approximately 7:23 p.m. on September 12, 2009, Employee #1, a swingman, was working at a port loading and unloading ships. Coworker #1 drove up in a utility tractor rig, Kalmar, Model Number PT122, Serial Number 5706, Ports America Number 5955, to Lane Number 2. Coworker #1 stopped at Coworker #2, a signalman, signal, and waited. Coworker #1 received the signal from Coworker #2 to go, and moved forward. Coworker #2 saw Employee #1 step into Lane Number 2 and was dragged under the back passenger wheels and crushed. Employee #1 was killed.
