Investigation Summary
On November 29, 2008, Employee #1, an employee of Fed Ex Ground, a courier service, was performing preventative maintenance at conveyor LCF2-1. The conveyor belt was "off," when the conveyor started up and pulled in the air gun that Employee #1 had been holding in his right hand. He sustained two broken bones and lacerations in his arms. He was hospitalized. There were other employees in the building, but they were not witnesses.
