Investigation Summary
On September 22, 2009, Employee #1 was using a Makita 9557NB Serial Number 0202812Y, which had an inappropriate saw blade installed. Employee #1 was working on a step ladder, by himself, completing a patio cover installation. Employee #1 was cutting the fascia board on a residential house when the Makita angle grinder rebounded into Employee #1's face, cutting his face and just below his right eye. The homeowner called emergency response, and Employee #1 was taken to County USC Hospital, where Employee #1 received stitches and was released to home.
