Investigation Summary
On September 2, 2008, Employee #1 was cutting two pieces of wood with a radial arm saw. He noticed that the two boards were not even against the stop. The saw was on, but in the recessed position. The employee placed his hand in the middle of the boards to flush them against the stop, when his gloved hand was caught by the teeth of the blade. The rotating blade pulled his left had into the saw, partially amputating his middle finger, fully amputating his index finger, and severely lacerating his thumb. Employee #1 was hospitalized.
