Investigation Summary
On June 27, 2008, Employee #1, a supervisor, needed to gain access to a vibrating air pipe line secured to roof trusses above the floor of a manufacturing plant. A coworker stacked boxes of bottle caps on pallets to a height of approximately 16 ft on the forks of a forklift. Employee #1 got on top of the makeshift platform and was raised up to reach the vibrating pipe. As he was securing the pipe, the stack of boxes tipped over. He was thrown from the platform striking his head on the concrete floor. Employee #1 was hospitalized with a concussion.
