Investigation Summary
On August 26, 2008, Employees #1 and #2 and two coworkers were working in an excavation that was approximately 12-ft deep, 40-ft long and 4-ft wide. The employees were working at the bottom of the excavation when a strong wind blew a section of pipe into the excavation. The pipe fell on the employees while they were trying to exit the excavation via a ladder. Employees #1 and #2 were severely injured, and two coworkers managed to escape without injury. Henderson Fire Department was then called to rescue the employee and later they were transported to the hospital. They were hospitalized and treated for multiple unspecified fractures.
