Employee Falls, Fractures Neck, and Dies

On August 12, 2008, Employee #1 was installing a copper roof of a cupola on a single story outbuilding with storage above. He was improperly utilizing fall protection equipment to protect himself. He had wrapped and then tied a rope around the cupola. He then attached the snap hook located on the end of the rope to the D-ring on the body harness. He did not use the rope grab or lanyard which was located in the outbuilding along with 3 other complete fall protection kits. He left slack in the rope. He fell approximately 17 ft, striking the hard compacted soil. He suffered a broken neck and died.
