Investigation Summary
Employee #1 and a coworker were removing a light fixture while Employee #1 was standing on a 10-ft step ladder straddling the top of the step ladder. The employees were removing the fixture and the existing pole and were going to replace the existing pole. The Employee #1 went up the ladder and, in the process of removing the 40-pound light fixture from the pole, started to straddle the ladder with one leg on each side of the ladder. At times, Employee #1 was sitting on the top of the step ladder. Employee #1 completed removing the light fixture from the pole using both hands. Employee #1 fell over backwards from the ladder and struck the concrete ground. The fixture landed on top of Employee #1. Employee #1 suffered unspecified injuries and died days later at the hospital.
