Investigation Summary
At approximately 12:15 p.m. on May 2, 2008, Employee #1, a warehouse worker of 4 State Trucks, used a Hyster Ordermaster RS30, a battery powered material handling piece of equipment, which could elevate an employee to retrieve parts from elevated storage areas. Employee #1 used this equipment to retrieve a new seat for an over-the-road tractor truck from a pallet rack located twelve ft above the warehouse floor. The elevated work area consisted of a short work platform, with two short hand holds, part of the Hyster, and two pallets stacked on the forks Employee #1 and his coworkers used to walk on and retrieve parts. The top pallet had a sheet of plywood on it to provide a level platform. Operating controls were immediately available to Employee #1, along with a body belt and lanyard, which was attached to an overhead anchor point. For some reason, he lost his footing and fell. Emergency response was called. Employee #1 was transported to St. Johns Hospital in Joplin, Missouri for treatment, where he later died due to the injuries he sustained in the fall.
